The promotion is available to all DBS PayLah! users who scan to pay at participating merchants in Malaysia, Indonesia, or Thailand using the respective local QR payment networks:

Your first eligible overseas QR transaction each calendar month receives 100% cashback, capped at S$3. The cashback is credited to your PayLah! wallet on the same day as the transaction. Using PayLah! for overseas QR payments is no more complicated than paying locally. One useful feature is that you see the SGD equivalent before confirming, so there are no surprises at checkout.
PayLah! applies a 2% markup above the prevailing wholesale interbank rate for currency conversion. There are no additional transaction fees beyond this. For most small purchases (meals, coffee, convenience store runs), this is a reasonable spread, and the S$3 cashback on your first transaction each month effectively offsets it on that payment.
For larger purchases where you are more sensitive to the FX rate, it is worth benchmarking against your travel credit card. The DBS Altitude Card earns 2.2 miles per S$1 on overseas foreign currency spend, which at typical valuations of around 1.5–2 cents per mile represents a reasonable return. The promotion requires no registration and no minimum spend. The monthly cap of 11,100 redemptions is worth noting. This promotion has been running in prior iterations, and while 11,100 slots sounds adequate, it is a first-come, first-served arrangement. If you are travelling early in the month, this is unlikely to be a concern. If you are travelling in the final days of a calendar month, there is a small chance the cap has been reached. With three months of the promotion remaining (April, May, and June 2026), the maximum cashback available to a single user is S$9, assuming one eligible transaction per month. Not a significant sum, but it is effectively free money for doing what you would likely do anyway.
